Up for giveaway is a mini helmet autographed by former NFL safety Matt Ware. Matt attended UCLA and was drafted by the Philadelphia Eagles in the third round of the 2004 NFL draft. Matt is a long time patient and friend of Dr. Liu’s.

(1) The winner must pick up the mini helmet at Dr. Liu’s La Puente location.
